Complaints concerning personal data processing
Filing a complaint Motion to file a complaint concerning the unlawful processing of personal data (This form can not be used to file a complaint within the meaning of the provisions of Chapter VIII of the Code of Administrative Proceedings)

Stamp duty and other payments
Stamp duty - paid in cash or by money transfer - is due for considering a case which may be related to issuing an administrative decision by the Inspector General for Personal Data Protection (PLN 10); the payment obligation arises at the moment of lodging a complaint with the Inspector General.
For submitting a document confirming a power of attorney or its excerpts or copies during the administrative proceedings (PLN 17); the payment obligation arises at the moment of submitting the document to the relevant public authority - cf. Article 6 section 1 para. 4 of the Polish Act on Stamp Duty.

Additional information
Motion on issuing an administrative decision by the Inspector General for Personal Data Protection can be filed in writing and forwarded to the address of the Bureau of the Inspector General for Personal Data Protection or in electronic form to the address of ESP-GIODO (Electronic Inbox). Sending a motion by electronic way is possible directly after the completion of the electronic form. It is important to describe the subject of complaint in brief and unambiguous way and to present the expected actions to be taken.
